Treatment: To provide, coordinate, or manage your care and related services, including consultation with our supervising medical director.
Payment: To bill and collect payment for services provided to you.
Healthcare Operations: For internal administrative purposes, quality assessment, and staff training.
As Required by Law: When required by federal, state, or local law, or in response to a valid legal process.

We will not use or disclose your health information for marketing purposes without your specific written authorization.

The right to request access to and a copy of your health records
The right to request corrections to your health information
The right to request restrictions on certain uses and disclosures
The right to request confidential communications
The right to receive a paper copy of this Notice upon request
The right to file a complaint if you believe your privacy rights have been violated

4. Our Responsibilities

We are required by law to maintain the privacy of your PHI, provide you with this Notice of our legal duties and privacy practices, and abide by the terms of this Notice currently in effect.

We reserve the right to change this Notice and to make the revised Notice effective for health information we already have as well as any information we receive in the future.

6. Complaints & Contact

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
